How can I tell her she needs to work on her looks without offending her?

My friends is a sweet and lovely person but, I'll be honest, she is ugly.
I want to help her look better but I dont want to damage her self esteem.
How can I suggest she works on her looks without hurting her feelings?
She keeps asking me why she has never had a boyfriend (she is 24) ,is it her personality or her looks, and I dont knwo how to break it to her.
